If you have Support Coordination in your plan, you’ll work together with your Support Coordinator to unpack your plan, decide how to spend your funds to get the most value, and choose the providers and services that best meet your needs.
We know that in a rural and remote area there may be limited services and long waiting times. One of the most important things your Support Coordinator can help with is to think outside the box and link you into alternatives while you wait. This may include things you can do at home, online programs, telehealth or visiting services, amongst other options.
At Realise Ability we provide all levels of Support Coordination.
Our Support Coordinators are qualified professionals who have lived experience of the NDIS. As we are locally based, we provide face to face support to help you understand and get the most out of your NDIS plan. The Support Coordinators are able to work with you to set your goals, work alongside you with all service providers and help your plan funds go further by identifying and connecting you with existing mainstream community resources.
We are available for you to pop in anytime you have a question.
As a provider of both therapy services and support coordination we are required to have a robust conflict of interest policy. We will never, only recommend our services. Your Support Coordinator will provide you with information about ALL the therapy and support services that are available to you, not just Realise Ability. Please ask to see our policy at any time and ask us how we manage it. If you have any concerns that your Support Coordinator is not working in your best interests, you are welcome to discuss your concerns with us or lodge a complaint via the NDIS Q&S Commission
