Realise Ability is a therapy and support service that supports you to achieve what you want in your home and community. We have support co-ordination, core supports and capacity building services which you can use an NDIS plan a GP management plan or your private health insurance.
Our team work within a social model and human rights model. We provide a wrap-around and holistic service that helps you work towards goals that are important to you, in environments that are important to you. You set the goals that you want to achieve and work towards these goals with support to remove barriers and learn new skills. As the barriers are removed, mainstream support is increased and you will build capacity which will in turn reduce the support you need.
We can build and support your team, to support you if you have a goal you want to achieve. This may be at home, at the beach, in the pool, in sport, at school or anywhere else that is important to you. The therapist is important, but not as important as you and the rest of your team.
Your team may include family, teachers, friends, coaches and anyone else who plays a role in your life. Every now and again, you will need to check back in with your therapists to make sure you are on track and get some new ideas.
You are the expert and your life and your goals dictate where you practise your skills.
You choose the goals – your therapists help you work out which skills and strengths you need to develop to achieve these goals and together you can write a plan for your team.
You choose where you want to use the skills – your team will work with you in these places to ensure your new skills are useful and functional.
We know that our services are a little different to what you are used to and might not be suited to everyone but that’s OK. The best thing is that you can pick and choose the services that suit you. If we are not right, we can help you find a service that is.
Our team is made up of local therapists and external professionals who visit us to help provide a holistic team approach to your therapy.
Our Psychologist will work with you around goals related to social skills, anxiety, resilience, change, behaviour and relationships. They will not provide counselling, but help you problem solve and give you ideas and strategies to explore and practice with your team. CURRENTLY RECRUITING
We also have a Clinical Psychologist visit us four times a year. With our local therapists, they run a diagnostic clinic and are able to complete Autism Diagnosis Assessments and Assessments of Cognitive and Developmental skills. They can also diagnose and provide support for learning disorders, dyslexia, dyscalculia, FASD and ADHD.
The Occupational Therapist at Realise Ability looks at daily living, play, work and employment, social skills, behaviour and relationships. The plan they build with you will include strength, coordination, emotional regulation, organisation skills and the confidence to use your new skills every day.
Speech Pathologists assist people to develop the communication, eating and social skills they need to have successful relationships, participate in their community and to build their literacy and numeracy. A speech plan will involve practicing speech, language and communication in a variety of environments.
Strength, coordination, fitness and mobility are areas where Physiotherapists and Exercise Physiologists may guide your team to plan and support fitness, sport and physical activities with you.
Our Dietitian works with our feeding team (speech pathology and occupational therapy) to ensure our clients receive adequate nutrition to achieve their goals, whether they are physical, social or emotional goals. CURRENTLY RECRUITING
We have a Key Worker, who is a qualified teacher. She works with clients and families to plan and coordinate their Realise Ability journey. She also loves working with anyone who needs some additional support to transition goals to home, school and other community locations.
The Therapy Assistants at Realise Ability have formal qualifications and are trained by the therapists to ensure they are skilled and qualified to help you maximise your plan. They will help you turn the goals you set with your therapists into reality. They work closely with your therapists to help you practice your new skills in everyday environments.
Our support workers are called Mentors! These amazing people can help you build your capacity in any area of life. You tell us what you want to achieve and where and our mentors will help you break it down into steps and then work with you to make it reality.
